Electronic Personnel Action Form​

An EPAF is the way in which we process employee status changes.  Instead of using a paper form to hire, end a job, or make changes to an employee, the use of an EPAF can be more efficient. ​

EPAF Originators & Approvers​

All employees requesting access as either an Originator or an Approver must complete the SUS online EPAF training.  You will need your sus credentials to access these trainings:​
