SUNOMA | Event Approval Request Form

Complete the form and submit it to the Museum Director at least one (1) month prior to the proposed activity/event.  

All fields are required; if not applicable, mark N/A.  

All acronyms must be spelled out.  

 

Per the SUNO Events policy, the Museum Director reserves the right to deny requests not received two weeks in advance.   

 

If this is an official SUNO Event, once the Museum Director approves of the SUNOMA Event Approval Request Form, please fill out the official SUNO Facility Rental & Campus Event Form. The form can be found on DocuSign.
