Audio/Visual Equipment Request Form

This form is for internal use only. Facility renters are responsible for providing their own A/V equipment, which must be approved by the Office of Communications.

 

**Submitting a request does not guarantee that your event will be covered.**

Request must be made 2 weeks in advance.
(time of event should allow enough time during the work day to set up and break down) 9am -4pm

Live Streaming, and/or photography cannot be provided simultaneously as it takes multiple people to complete this request. *Live streamed events must be streamed through the University YouTube account,and prior approval is required from the Director of Communications.

The Office of Communications staff are the only approved personnel allowed to operate ANY EQUIPMENT on campus.
